Transportation charges...

Dear all,
Our centre also provide transportation services to pick-up patient at their home / office to centre and send them back after treatment.

The charges are as follow :

1. RM 0.90/km x 2 (to and Flor)

example : Patient A from home to centre distance 10km
               10KM x 2 x 0.90 = RM 18/ treatment
               1 week = 3 treatment session = RM 18 x 3 = RM54.00
               1 month = 12 treatment session = RM 216.00

This will be charges after every treatment session and by cash / cheque. This service charges will be separated from JPA / SOCSO / INSURANCE claims.
